Community Care Initiative FAQs
Who qualifies?
Anyone who is part of the community group we're currently featuring. Each month recognizes a different profession or role. Both new patients and existing Back To Health patients from that group are welcome to participate.
What does the visit cost?
New patients in the honored group get our $56 New Patient Special — consultation and exam, digital X-rays if clinically needed, and your report of findings. That's it. No deposit, no hidden fees, and nothing due before you arrive.
Is there a deposit?
No. There is no deposit and no prepayment. You pay $56 at the clinic on the day of your visit.
Can I nominate someone?
Yes — and we love when people do. Use the Nominate Someone form to tell us about a coworker, friend, or family member who deserves to be recognized. We'll reach out gently and let them decide if it's right for them.
Can I nominate myself?
Absolutely. If you're part of the group we're currently honoring, just complete the reservation form.
How do I reserve a spot?
Complete the short reservation form and choose the clinic closest to you. You'll be sent straight to that clinic's online scheduling page to pick your time, and our team will confirm.
Which clinics participate?
All three — Zeeland, Portage, and Muskegon (Norton Shores). Pick whichever is closest on the form.
Do I need insurance?
No. The $56 special is a straightforward self-pay visit, so insurance isn't required. If you'd rather use insurance for ongoing care, we'll explain your options at your visit.
What if I'm not part of this month's group?
You're still welcome. The $56 New Patient Special is available to any new patient at all three clinics — the monthly spotlight is simply our way of saying thank you to one group at a time.
How long is the visit?
Plan on about 45 minutes. You'll get an exam, X-rays if clinically needed, and a clear explanation of what we find.
What should I bring?
Comfortable clothes, a photo ID, and any recent imaging or records you'd like us to review.
Will I get sales calls afterward?
No. We'll follow up once about scheduling and that's it. No pressure, no marketing calls.
