What's usually driving it
Tendons adapt to load slowly. A jump in running volume, a change in footwear, or years of limited ankle motion pushes the tendon past its repair capacity. The collagen becomes disorganized rather than inflamed — a degenerative state called tendinopathy — and it can persist for years without the right stimulus.

Achilles Tendinopathy: common questions
Should I rest until it stops hurting?
No. Complete rest weakens the tendon further. The evidence supports carefully loaded rehab — some discomfort during exercise is acceptable, and we'll define exactly how much.
Can it rupture?
Chronic tendinopathy carries some increased risk, which is another reason to treat it rather than push through it. A sudden pop with inability to push off needs urgent evaluation.
Is insertional Achilles pain harder to treat?
It's slower and needs different loading — less deep stretch, more controlled range. Shockwave is particularly helpful for insertional cases.
Do I need a referral to be seen?
