The shoulder is the most mobile joint in the body, which means it depends on muscle control rather than bone structure for stability. When the rotator cuff or the shoulder blade stops doing its job, tendons get pinched under the bone above them — and reaching overhead becomes a project.

What's usually driving it

Most shoulder pain is a mechanics problem before it's a tissue problem. If the shoulder blade doesn't rotate properly — usually because the mid-back is stiff and the scapular muscles are weak — the space for the rotator cuff tendons narrows every time you lift your arm. Those tendons get compressed thousands of times a week until they become irritated and, eventually, degenerative.

Shoulder Pain & Rotator Cuff: common questions

Do I have a tear?

Small degenerative cuff changes are extremely common and often painless. Our strength testing tells us whether the cuff is functionally intact; if testing suggests a significant tear we refer for imaging.

How does shockwave help a tendon?

The acoustic waves increase local blood flow and stimulate a healing response in tendon tissue that has stopped repairing itself. It's especially useful for shoulders that haven't responded to rest and exercise. See our shockwave therapy page for details.

Should I rest it completely?

No. Complete rest stiffens the joint and weakens the cuff. We modify what aggravates it while keeping you moving in pain-free ranges.

Do I need a referral to be seen?
